Personnel Activity Report (PAR)
Instructions for completing the PAR form.
Review payroll percentage--if incorrect by plus/minus 5 percent correct
PAR and do Payroll adjustment (UPAY646). Copy should be attached.
Reporting effort must equal 100%.
If pay is directly charged to a sponsored project, there must be some
effort reported under this category. Effort percentage under sponsored
projects must be at least equal to the pay from sponsor project
percentage. If there is cost sharing on an award, the P.I. should
have some effort reported on the PAR under Sponsored Projects.
Certification of effort is required to verify work performed and the
distribution of effort as reasonable. A responsible official should
sign for non-professional employees (employee signs only if the official
does not have sufficient knowledge of activities). A staff employees
effort should be certified by the MSO, PI, chair, or immediate supervisor.
A professional employee can sign their own PAR or a responsible official
should sign. A responsible academic official may sign for faculty or
academic appointees if the official has a knowledge of activities.
Faculty and academic appointees (Title Codes 0000-3999) usually sign
their own PAR.
If salaries are charged to two different departments grants then both
departments must sign.
